Lets face it hit point damage happens but Its not the only way.

Death by massive damage.
Being too far from your original body when the Magic Jar effect ends.
7. Drowning
8. Plane shift to Elemental Plane of Fire
9. Transformed into a non-living being.
Piss off the one unhinged player at your table who identifies too closely with his character. Mess with said character. Be smothered in your sleep after the game by said unhinged player.
13. Failing both saves against phantasmal killer.
15. Ghoul Fever
16. Kissed by a vargouille and failing all the saves.
19. Fail the fort save when break enchantment is used to 'unstone' you.

44. Leave a timeless plane and suffer the retroactive effects of a lot of time passing at once (e.g., instantly aging into dust, suddenly dying of starvation because you skipped a year's worth of meals, etc.).
45. Use create pit to drop someone into a hole, then cover the top of the pit with stone shape. When the create pit spell ends, the floor rises back up and the victim is crushed against the new ceiling.
Imprisonment might be worth an honorable mention - it doesn't kill you, but it can certainly cause you to lose a character if you don't have a way to reverse it.

'Hit by a speedboat' is something one of my GMS say kinda like 'Rocks Fall everyone dies'...but I guess more single target.
46. Eaten by a grue.
47. Smited by GM for trying to use create pit and stone shape to auto kill enemies.
Play a VERY long campaign and have your PC die of old age.
50. Baleful Polymorphed into a fish on land.
51. Get Bullrushed by a Shaitan into a stone wall (involuntary Meld into Stone), then have it cast Transmute Rock to Mud on you.
52. Drink too much Midnight Milk and have that one dream...
53. Getting Flesh to Stoned while Astral Projecting.
54. Fail Sense Motive against the BBEG and help him end the world.
55. Getting too friendly with a Yellow Musk Creeper patch.
56. Altitude sickness while lost in the mountains.
