When a creature like a stone giant has 2 slams +16 (1d8+8), does that mean he gets 2 separate slams at +16 or just 1 slam that flavors like the PC or whoever is getting hit twice?

pretty sure it means that it has 2 slam attacks it can use during a full attack.

2 attacks that use the same stats.

Its two Slam attacks at:

+16 (1d8+8)
and
+16 (1d8+8)

Note that you can flavor Slams however you want, as they sort of aren't directly attached to any particular limb (for natural attacking purposes).

Just remember that it is for a full attack like Karjak said. If they only have a standard attack they only get 1 slam attack.
