1. I have 2 claw attacks and a bite and I am wearing a boulder helmet
can I make a 4th headbutt attack with the Boulder helmet?
The Dwarven Boulder Helmet is a light melee weapon.

2. If I can"t make the 4th attack why not?

3. If I can make the 4th attack what is the to hit with the helmet.

4. If I have no feat for use of a boulder helmet what is the penalty to attack

5. does wearing a weapon automatically allow you to use it?

6. Can you use a dwarven Boulder helmet if you are not a dwarf?

1.)No

2.)You cannot make a natural weapon attack with a limb(bite and head in this case) with which you make a manufactured weapon attack (the boulder helmet) during the same attack action.

3.)The boulder helmet is a manufactured weapon, and has the same to-hit as any other light manufactured weapon.

4.)Same as any other weapon for non-proficiency, -4

5.) Yes, its just a weapon

6.) Yes, dwarfs just treat as a martial weapon instead of exotic due to their weapon familiarity trait

7.) You may substitute a headbutt attack for a great axe attack, but you would not be able to use two-weapon fighting with a great axe and the helmet to gain extra attacks above those granted by your BAB.

Grand Lodge

1) I think so, but it will do nasty things to your claw/claw/bite attacks.
2) Might be in conflict with the bite, since you use your head to make both attacks, so it might be bite or headbutt, not both at once.
3) Helmet uses your BAB plus your Str mod, Dex mod if you have Weapon Finesse. It does +1x Str damage. Note: Your claw/claw/bite all take a -5 penalty because they all become secondary attacks, and also only do half (.5) Str damage.
4) -4 for non-proficiency, like any weapon you don't have proficiency with. Depending on how it is classified, it might fall uunder another proficiency, although it sounds like it might be an Exotic Weapon, offhand.
5) No. Wearing a sword, for example, doesn't automatically allow use without unsheathing and wielding it. The helmet, on the other hand, probably threatens when properly worn.
6) If your GM allows, yes. Campaign rules can vary.
7) As long as you take the proper Two Weapon Fighting penalties, or make only the number of attacks allowed by your BAB. There is an FAQ entry on this part of it. Fighting with Two Weapons versus Two-Weapon Fighting

1) Yes, you can. Unfortunately, this means that all natural attacks become secondary natural attacks, and are made at -5 to hit and are get only half of the strength modifier or damage. On a side note, a head is not a limb, and it doesn't appear to follow the one attack per limb logic. Take a look at the gargoyle monster profile for instance, and you'll find a creature with a gore attack and a bite attack, but only a single head. One mouth though, can't have two bite attacks as shown in the toothy/razortusk faq.

3)The helmet takes the role of the primary hand weapon in this instance, so there is no penalty and you get the full strength bonus to damage.

4) if you aren't proficient with the helmet, you suffer a -4 penalty to hit. Mind you, there are other ways to become proficient with an exotic weapon outside of feats.

5) I don't really follow this question. You can use almost anything as a weapon, but it doesn't mean you'll be good at it.

6) Yes, as stated before, it's easier for dwarfs to gain proficiency, but for instance, there is an alternate racial trait (ancestral arms) for half-elves that gets them proficiency in any exotic weapon.
