Witch Docter; effects that specifically cause pain and what is in a mask?

Maybe i am reading too much into this but wouldnt anything that causes HP loss constitute a specifically painful effect? Seems really powerful but also sort of inline with the loss of a mobile familiar and all of the associated familiar feats/abilities. (refering to the +2 on saves versus effects that cause pain OR have the pain descriptor)

The other odd line is "At 5th level, the scarred witch doctor gains the ability to add magical abilities to her mask as if she had the Craft Wondrous Item feat" ... So the mask occupies the head slot, which is the purview of specific function items. If your mask is made to mimic say a Jingasa of the Fortunate Soldier i take it you could not later add another specific helmet effect onto it? Is there a system i am forgetting which would allow you to disenchant it and apply a stronger effect at a later level? If the mask is destroyed and you make a new one, is the magic effect included in it or must one make a seperate craft check to re-enchant it?
