Half Dead City Falling Apart

I recently received Half Dead City (AP 79). Yesterday, while reading it, a section of the book fell out (pages 1-14).

This is the first time that I have ever had this happen - is there any way I can get it replaced?

[Edit] - I meant to add that my copy of Half Dead City was from my subscription, not from a 3rd party retailer.
