This order was created on October 7th and was supposed to be shipped by October 21st. However, I have never received a shipment notification and the order has not been received as of Friday.

That was the missing order and I hadn't previously received the shipment notification. It appears that someone decided to repackage it in a large box instead of the normal shipping envelopes. I suspect that this was the cause of the problems.
