Lots of elemental items. By which I mean, items that have properties of all four standard elements in one item (though not necessarily simultaneously).
What's wrong with picking one element and sticking with that?

Time magic makes GMs hate you.
More than half your word count is used for description of the item, but its actual function is described in less than half that.
How did they say to report IP theft? I just found one ripping off League of Legends quite specifically but it may be subtle enough to pass by.

I just have to share a facepalm moment. The very first item I read, on my first vote... is a blatant violation of Rule #22. "This thing can check for traps!" (paraphrased, but that explicit).Yeah, but I think I wound up voting for that item because it was the better of the pair X_X

Just saw an item full of old school flavor and kind of fun, if whacky, mechanics. Unfortunately, what it actually does is rather bland.
Alliteration is not cruise control for cool and witty!!!!

If there is anything that ANY fantasy setting does not need, it is auto tune.
And yet, the other item manages to be worse.

Ow. I'd forgotten how much this makes my head hurt. Choice between "Bad grammar, unclear rules, too short for what it does" and "I don't even know... but it's definitely too wordy and metagamey."

Ok that one could have been cool and not over-powered at all, if the writer had not put in a bane. No reason for the bane if the use is that limited.

I've encountered two items that are the same idea as each other. Different slots and (slightly) different details, but basically the same item. Awkward.
At the same time, or just separately encountered them? Because it would be even more awkward for the people who get the two put head-to-head...
That said, I haven't run into my item yet but I've found one with a similar theme (but very different mechanics). I had to double check that it wasn't mine and that I actually submitted what I thought I did.

gross out items annoy me, light based items also annoy me... both are otherwise fine just not creative. coin flip?
If you're going to price something over 200,000 gp, please at least make sure it's actually worth that much (even though it really should be an artifact).
