Since my party is made of experienced players, I made Deradnu a full Half-fiend minotaur. He rolled good on the initiative and smited + full-attacked the inquisitor, killing him. He was close to doing the same to the tiefling paladin of Sarenrae (even without the smite) when Irabeth killed him.

The Gory Details:

The party of 7 acknowledges that there are two ways to enter the Gray Garrison, and Poppo wisely decides to ask the Spirits for guidance and asks for an augury.
"What will be our fate if we enter from the shutter on the second floor?" The result is a clear "Woe!", indicating that entering from the main entrance is a better choice.
Regardless, after defeating the enemies in the courtyard, Poppo uses the scale of levitation on himself, followed by invisibility, and tries to enter the shutter. Alone.
The rest of the party, not knowing where he went, opened the main door and fought their way through the first tieflings, while Poppo eventually opens the shutter and drops down there. There were six demonic giant flies there (since the party sports 7 members, I routinely double the encounters' worth of xp), the party already fought five of them while exploring the ruins of Kenabres and everybody knew they weren't just regular giant flies, including their ability to spit acid and use darkness. Poppo, while invisible and aware of the flies, OPENED THE AERIE WINDOW, alerting them (they have an INT of 10 afterall). Not seeing anyone, the flies (they beat Poppo's initiative) moved and spitted their (unlimited supply) acid in the off chance that an invisible enemy was there, and they almost killed him. Poppo stays put and DOESN'T move, hoping for the flies not to have noticed (despite the outline that his invisible form made when hit by the acid), so the flies proceed to cast darkness and surround him. Poppo, effectively blinded, tries to levitate upwards while using acrobatics, alas his speed while double moving is just 10 ft against the flies' 60 ft. They swarm and murder him next round, his invisibility not helping much against 2 new acid sprays and a lucky bite that manages to get past his concealment.

Konneshka rolled the 20,20, confirm and proceed to rip Hado head off his shoulders while the rest of the group fought off the other demons. After the party defeated and explored Southbank in Drezen, they found themselves assaulted by an angry mythic chimera. Seeing their comrades in danger, Kairon tried to attract the monster' attention, successfully. First turn, Soltengrebbe breathes as a swift action inflicting some damage to half the party, then moves and gores the paladin, tripping him.

Second turn, Soltengrebbe full attacks the prone paladin, ripping him in half. The next round the inquisitor almost got killed too, but Soltengrebbe died before finishing him off.

Note: I used my version of Soltengrebbe, wich is significantly harder than the original.

The Gory Details: I upgraded the fiendish baddie to a full half fiend because my party is simply that strong. Well he charged in, took an AoO for about 1/5 his HP, then next round took a crit from a large great axe, followed by a 2nd attack . . . which also crit. Dropped him in his tracks.

Catalyst: A Dominated Paladin(PC) who crit using a bastard sword using Improved Critical(Bastard Sword) and Maximized Critical(Champion Power).
The Gory Details: In my campaign Arueshalae had fled during the last book after an unfortunate accident involving a Confusion spell and her killing Arken's(Female Paladin) cohort. I had her become captured by Minagho after fleeing to the Midnight Isles because the PCs hadn't moved quickly enough. The Paladin had fallen in love with the redeemed succubus and was over joyed to meet with her again in the Isles. What she got instead was a face to face meeting with Minagho pretending to be her lover by herself while the rest of her party slumbered.

This led to Minagho using Profane Wishcraft and some trickery to get the Paladin to wish for something under the guise of a friend and lover. The Paladin made mentioned that all she wanted was for Arueshalae and herself to be together once again. Which i responded to with "Is that what you WISH my dear?" which she promptly said yes to. She also failed her will save and became Chatic Evil thanks to Profane Wishcraft lossing all Paladin powers. After this she was easily dominated and brought back to Minagho's hideout to finish the corruption and create Staunton Vhane 2.0

The rest of the PCs used Scrying upon the Paladin when she hadn't returned when promised and went in to save her. The first on the scene was Tyk to the rescue he burst into the room and ravaged some of Minagho's incubi minions. After this the Dominated ex-paladin went and scored a natural 20, confirmed the crit and dealt around 200 damage to him killing him.

After that the rest of the party came in and cleaned up. But it came very close to coverting the Paladin to Baphomet using torture.

Our would-be hero moved aggressively towards Hosilla, forgetting or disregarding she was wielding a glaive (and thus had reach) and that she casted True Strike during her previous turn.
When Hosilla saw the half-orc approaching, she took the chance to strike (AoO) with decissive and calculated aim (a 20 in attack roll, ofc confirmed with the aid of True Strike). The glaive impaled neatly and swiftly the neck of Tyrus, dropping him dead instantly (28 damage).

The Gory Details: The party heard him coming, having not retired for the night. The paladin, being a paladin, charged off into the darkness to face this foe - followed swiftly by his Fighter/Oracle companion. The Bodak, a fallen crusader himself, of course focused his attacks on the smiting paladin.

3 saves later - rolls of 2, 1, and 4 - the holy knight of Ragathiel is down 6 negative levels, and eagerly awaiting his return as a new Bodak.

(The player forgot about his surges, but even rolling a surge die onto the last save wasn't enough to save him).

The party found themselves surrounded by enemies in Area F17 on their second assault of the citadel. Staunton laid a trap for them and waited until his minions had weakened the enemy.

Then he proceeded to kill a character each turn with his full attacks and additional attacks (Kurt was the first dead, but got revived by Kairon mid-fight with relentless healing). Four attacks + high bonuses to hit and damage + Nurah's bardic music = killing machine.


The PCs will be resurrected after the fight.

Note: I used my version of Staunton, which is significantly harder than the original.

The Gory Details:

Note: Nesteruk, Poppo and Cimaru are all PCs of the same player...
Since the party sports 7 people and they all are about 30 point buy, Deradnu has been advanced to a Fiendish Minotaur Barbarian 2 with maximum hp.
While beating up on Deradnu, Chimaru is brought into the negatives. After being healed to positive hit points, he attacked right away and Deradnu decided to end his suffering permanently.

After this, the party got to destroy the wardstone and Poppo came back to life!

The Gory Details:

The party got to fight against 9 ghasts and 2 ghasts clerics of Kabriri 4 on desecrated ground. Svarov went too far from the rest of the party and got surrounded by one of the clerics, his summoned babau and a couple of ghasts. A few rounds later, the cleric was about to be destroyed, but it remained at 1 hp and could not safely retreat from Svarov, so it instead full attacked in the off chance to paralyze the tiefling and move freely the next round. He hit 3 times, Svarov rolled a 1 on one of the saves, and one of the other ghasts delivered a coup de grace to the reckless tiefling.
The party found the healing cache beneath the altar, and it also included a scroll of Raise Dead, which they promptly used on behalf of their friend (but risking scroll mishaps twice as the party bard was failing to read it)

The Gory Details:

After being raised from the dead, Svarov had 2 permanent negative levels.
Maugla got some growth points to make up for the more powerful party and was hiding in deeper darkness and a silenced area. Svarov was the FIRST to enter the area, alonem despite his weakened condition, and got hit by an enervation that ate 3 more levels.
1 level away from utter death, Svarov didn't get the hint and stubbornly went to melee range a few rounds later with other party members. Maugla activated his death-stealing gaze and Svarov died for good.
With no ability to raise people from the dead again, Svarov is probably being replated by a (much needed) cleric, as the Mythic Power is passed to a poor prisoner nearby that Nulkineth kept for dinner.

The shadow demon charged the dwarf, who was already hurt because of the mythic fireball the aasimar sorceress casted while possessed by the demon. After 5 successful attacks (with one critical hit), he was dead. The paladin used relentless healing to bring him back on the same round.

The Gory Details: While battling Mutasafen and Minagho, the PCs cornered them in Minagho's quarters. The lilitu used her dust of disappearance to vanish while Mutasafen continued to fight. Not wanting her to get away, Illuren sealed the exit to the chambers with arcane lock. Unable to pick the lock or break the door, Minagho had no choice but to attack. One full attack with Power Attack, sneak attack, and a few criticals brought Illuren from full health to dead. The next round, she did the same thing to Variel. With no easy way to get through the dust of disappearance, the party mage hit on the idea to use major creation to fill the room with flour! Thus revealed, the others were able to deal with the murderous demon. Illuren was brought back with oil of life, and used raise dead to bring Variel back as well.

The mythic heroes found the woundwyrm's lair west of Drezen and entered the cave. After passing through a rift created by the chaotic energies of the wyrm, wich dispelled some of their buffs, the blind dragon appeared from another rift and the battle started.

During the fight, both the paladin and the inquisitor were killed due to the enemies' overwhelming physical attacks. Even the sorceress was close to dying after Scorizscar focused on her to end those pesky damaging spells. But the dwarven cleric was fast enough, and after several mythic breaths of life, mythic cure critical wounds and channel positive energies, both characters were back in the fight.

In the end, exhausted and drained of mythic resources, they were able to slay the mighty beast.

Note: I used my version of Scorizscar, and added two environmental effects to the fight.

Gory Details:

Kurt and the rest of the PCs fought valiantly against Skullgrym and his undead horde, but he was always harassed by Alrys and another fallen of the family. After helping the group survive negative levels and damage in general, he died after taking a crit to the face from his own ancestor.

His soul was consumed by Skullgrym, who was eager to taste this so-called "mythic power".

Note: I used my own version of Skullgrym and a very different encounter setup.

Today we lie to rest a great hero of the Crusade, Sir Magnus. Inquisitor of Iomedae, Savior of Kenabres, The Reclaimer of Drezen and the Sword of Valor, Knight Commander of the Silver Talons, Lord of the Silver Dragons, Defender of the Gates. The bards will sing songs of his defiant defense of the Silver Dragon garrison for centuries. His heroic, no mythic, exploits will serve as a guide to those of us who follow in his footsteps, heroes all.

But though we all mourn the passing of such a great hero, leader, and friend. His death serves as a lesson to us all. That no matter what horrors the Worldwound gives birth too, that we will stand defiant, indomitable, and unbroken. For we stand at the gates of the world, the last line of defense from the horrors of the Abyss that seeks to wash over us in a flood of blood and terror, but like Sir Magnus, we will stand defiantly.

The horrors in the night seek to break us, they think robbing us of one of our greatest heroes will demoralize us, will defeat us. But I say it will spurn us forward, we will confront our foes and show them we are all like Sir Magnus, brave and unbreakable. We will stand and we will fight no matter what they send against us. For we stand on the precipice of history my brethren, for it will be us warriors who will finally end this threat to our world. For we are the Knights of the Fifth Crusade, we will heal the wound, and save the world.

For story reasons I kept Exorius for this part of the adventure. While the party was dealing with the Fane's guardians, Exoius came from outside, flying through the tunnel. After a charge with a lance and the Spirited Charge feat, along with the rift drake's pounce, Kurt was lying dead on the cavern's floor. He was later revived by the paladin, using relentless healing.

The party arrived at the Molten Scar, lead by a strange pull the inquisitor was feeling. All they could see was a huge cloud of energy and steam, crossed by lightning bolts. After crossing it, they emerged 30 ft. over the lava and the rock islands. After seeing a huge amount of vrocks doing dances of ruin to power up the ritual, combat started with Vorimeraak (who was conducting the ritual), and three vrocks.

On the first round, vorimeraak moved and full attacked (fleet warrior) Kiha, dropping her into negative hitpoints and making her fall into the lava (the fly spell requires a bit of concentration, so if you go unconscious...). The lava did its work and finished her off. She was later revived by the cleric, after the paladin's mount pulled her out of the lava, just before the body disappeared completely.

Note: in both cases I used modified encounters and statblocks.

Belorck and the rest of the party were battling Jaruunicka's minions to save Arueshalae. At one point in the battle, Beloc got heavily wounded due to some charges by the drake riders and a grappling hezrou. A group of three will-o'-wisps appeared near him, and after two hits he was dying on the ground. The third will-o'-wips coup de grace'd him, killing him instantly as he failed the Fortitude saving throw. He was resurrected a round later by the cleric, using a mythic breath of life.

The Gory Details:
Poppo dies for the 3rd time as he attacked Staunton for 50 damage and earned a full-attack retaliation with mythic power attack, smite good, and a x3 crit on the first round of battle for 240 damage.
The 7-men strong party is facing 4 half-fiend minotaurs, 7 babaus, and empowered versions of Staunton, Joran, Kiranda (who successfully bluffed and sneak-attacked the party at the worst moment) and Nurah.
The party used dimension door to surprise the enemy, but thanks to Kiranda's telepathic abilities, their opponents also had time to buff up like crazy and prepare well for the "ambush".
The fight is ongoing right now, let's see if more characters will join Poppo soon! Currently, both Staunton and Kiranda are locked against the two party "tanks" so hopefully there won't be more deaths.

The Gory Details:

Spoiler:
Millorn casted Color Spray on the poor paladin who failed his save. The round after, Millorn used a coup de grace to kill him.

The group arribed at Jerribeth's room and after a brief talk about them killing Xanthir Vang for her, combat started. She was already buffed, as the characters had been battling scions of Baphomet just outside her door.

On the her first turn, she killed the paladin after a full attack (one confirmed critical) + extra attack from divine power + rend. On her second turn, she killed the inquisitor the same way (another crit!).
Both of them were revived after that, and the paladin had time to bite the dust again before Jerribeth was killed by a spell from the sorceress.

Note: I used my own version of Jerribeth.

The Gory Details:

The party didn't quite realize how squishy the NPCs were, especially Anevia, as she had never taken a hit in the course of their entire campaign. So when the fighter, cleric, and sorceress split off to prevent the aurochs from pulling down the bridge, Barrid saw Irabeth, Anevia, Aravashnial, and the bard standing in a group at the end of the bridge.

Fireball #1 did low damage and the cleric came back to channel for them. Everything's OK, right? Fireball #2 was a massive 35 damage on 8d6. Anevia blew her save and was incinerated on the spot, right in front of Irabeth!

Even better for the story, Barrid Dimension Doored away just as Soltengrebbe arrived, leaving Irabeth and the bard alone in the middle of the bridge facing him. Irabeth got knocked to negative hit points twice, but the bard kept her alive as the sorceress got the fighter up in the air and going toe-to-toe with the mythic chimera.

The party used up ALL of their mythic surges, but finally brought the chimera low. As Irabeth held Anevia's lifeless body in her arms, the party voted unanimously to use their lone scroll of Resurrection to bring back Anevia.

Irabeth will be eternally grateful.

The Gory Details: Foremost to note is I've also been using Scorpion_mjd's statblocks.

Following the fight with the Gorgon, the Scions listened to the fight and hid to the sides of the porticulis and observed the illusion be dispelled. Seeing the gate was magical via arcane sight, Caladur made several attempts to dispel the gate, assuming it was a trap. After only suppresing the magic a few times, he decided that he would go up to the porticulis.

And was introduced to several glaives to the face.

The Cleric chose this as the day NOT to prepare Breath of Life, but the Oil of Life from Book 2 came in handy in this regards.

Ok, I'm posting this because this death should have happened, but as I forgot the spell was empowered, the character (barely) survived the attack.

As the door to Vang's room was closed, all the party members except for Kiha and the paladin's mount (lion) teleported inside using dimension door. Vang, who of course wasn't inside the room, approached the sorceress while under the cover of a greater invisibility spell. And then he casted disintegrate, and Kiha failed the save.

Would I have correctly applied the empowered effect to the resulting damage, she would have been into a pile of dust.

Note: I used modified encounters and statblocks.

Gory Details:

Humbolt got a bit too excited when the party saw Hosilla standing atop the table in H7. As the party's paladin chased after Uziel and the druid and sorcerer began to set up a flank on Hosilla, Humbolt stepped up and attempted to stab Hosilla with his longspear.

Hosilla the inquisitor, seeing her favoured prey just in range, took the opportunity to go for an easy advantage and stabbed at Humbolt. She hit extraordinarily well, getting a critical hit. With the x3 modifier from her glaive and the 6 damage bonus from a favoured judgment of destruction, she one-shot Humbolt from full to dead.

His poor brother the alchemist will be replacing him.

The gory details:
So after a long fought battle throughout the garrison, the party makes it up to the hall of first fatalities and face off against Deradnu. Given that they still had some big damage dealing abilities up their sleeves (namely in the arrows of evil outsider bane as well as the paladin's remaining smite use for the day) I gave Deradnu a level of barbarian to give some more hp as well as more use from the horn he carried. He managed to get a good hit in on our dear Lagos, before dropping the party's tiefling rogue to negative hit points.

As the party scrambled to save the rogue from an imminent death, Lagos copped a particularly vicious hit (read: nearly max damage, but not using power attack at the time) that took him from living to dead in one strike - the pain was quickly given back to Deradnu, as the paladin emerged from his shroud of invisibility to smite him, and the archer cleric fired off one his unidentified arrows, not realizing his good fortune that he had picked out a recently acquired evil outsider slaying arrow. And just like that, Lagos' death was avenged.

Lagos was subsequently restored to life by the wardstone's destruction, and is now planning a career as a Mythic hero.

After fighting some goristros, ash giants, and a horde of incubi fighters in the Fane's entrance, Ursathella appeared. Under the effects of mythic haste and other spells, she could move quite a lot. After trying to catch the sorceress, she got into a fight with the human inquisitor. He was already hurting, and after a few blows he was at negative hit points, but still acting due to Arueshalae's aura.

Enraged, Urshatella full-attacked, killing him with a critical hit (scythe crits hurt).

Ursathella died in the next round by the paladin's blade, radiance.

Note: I used my own version of Ursathella.

The Gory Details:

The party reached the room with the grinder and the portal after dealing with Ursathella and some demonic minions. Two goristros, one gibrileth, an invisible Minagho (projected as Yaniel) and Mutasafen (also invisible) were waiting for them.

During the fight, Mutasafen, buffed to the gills with mythic haste, mythic fly and mythic heroism, moved and then full-attacked. After taking a lot of hits from the bombs (and one/two critical hits if I remember correctly), Kurt was dead.

He was later revived by the paladin, who used relentless healing on him to trigger one of his higher-level healing spells.

The "Fickle Attack" mythic ability was amazing, it allowed Mutasafen to turn a lot of rolled 1's into 6's, upping the damage considerably.

Note: I used a modified Mutasafen, changing some of his feats so he could make more attacks.

During the first round, Shamira used Time Stop and summoned a balor. After the time stop ended, the balor targeted the aasimar sorceress with an Implosion. She failed the save and died.

The party wanted to fill the chalice of Ozem with some blood from the mythic nightcrawler, so they teleported to the cave below the breathless mountains. On the first round, The Father of Worms did a lot of damage to the party with its breath weapon after its shadow dispelled the death wards that all the PCs had. Then it casted a wail of the banshee. The cleric failed his will save, and couldn't surge due to being flat-footed.

Note: I used my own version of Odeenka, Baphomet golem and encounter setup. The same goes for the Father of worms.

Are you technically allowed to write your own character's obituary?

Simian and his group of 'heroes' had just cleared out the foundry, killing a man without skin and his bizarre fetal assassin and claimed both the suture (which is important, I believe) and a whole lot of treasure.

The storm king (our long absent friend who started this whole business) arrives and challenges us to a duel. Never ones to back down from a chance to absolutely steamroll all the mightiest creatures in existence we rushed down to the fight.

The sorcerer immediately cast mythic time stop, to allow us five rounds of buffing before rushing in to fight with the super balor. Although he did a healthy amount of damage to both me and the paladin we defeated him.

Part of my character's unfortunate personality is that he believes that both he and his companions are invincible, and he also represents a character who has been redeemed to an almost insane level.

While all the rest of the characters ran for their lives from the oncoming balor explosion I failed my save on purpose to prove my devotion to Sarenrae and that the Abyss' fires were nothing compared to hers blah blah.

I was dropped to -41 by the resulting explosion and... was raised from the dead automatically 24 hours later inside our Instant Fortress/Rod of Security demiplane. Simian's faith might be a bit shaken.

PCs dispose of vescavor queen handily but a vrock appears through abyssal gate with heroism and mirror images up. Valin lands a smiting critical hit for 75 damage even with the vrock still having 5 images up. Vrock hits Valin with 4 power attack hits. PCs steadily chew through its images. When Valin's turn arrives, there were only 2 images left. Valin lands 2 hits using his last hero point to get the 2nd hit, but only manages to destroy the last 2 images! Then the sorcerer tries a spell on it which would've finished it off (it only had 11 hp left at the time) but SR makes the spell fizzle. Then its turn comes up and it kills Valin outright with 2 power attack hits.

PCs use a raise dead elixir on Valin afterwards so the adventure continues.

During the taking of the Ahari bridge, the mythic dwarf champion Deek Blackhammer had fallen unconscious from a poisoned crossbow bolt (Nat 1). Awakening after 1 minute of unconscious rage, and receiving the Aasimar Paladin's laying of hands to remove his resulting fatigue, the champion buffed with fly as the Mythic beast of Drezen approached the Ahari gorge. On it's first turn it unleashed it's freezing breath on the party, though the dying gift of the silver dragon Terendalev warded most of it off. The dwarf was unprepared for the beasts second furious attack still groggy from the poison's lingering effects (rolled 4 initiative vs beasts 24/13).

Soltengrebbe (based on Sc8rpi8n_mjd 12 HD version, but added giant simple template) landed all 5 of its natural attacks,confirming 2 criticals, resulting in 200 damage before the dice were rolled... Using mythic power attack!

The party entered the vault and tried to overcome the "security system" to get the Herald's heart. They triggered the maze trap and Inger-Maggor appeared through a portal, ready to avenge his pet. Erum-Hel was already waiting, hidden and invisible, for the inquisitor of Iomedae an his pals to appear. He used his stealth capabilities to inflict three full-attacks on different rounds, killing the inquisitor each time with huge amounts of damage. One of these deaths came from the vorpal ability of Erum's rokha sword. The rest of the party watched horrified, as Beloc's head separated from his body... He was revived using miracles, mythic regenerate and mythic breath of life.

The Herald didn't want to be redeemed, and tried to kill the characters in his new lord's name. Since the other frontliner was invisible (g. invisibility + mind blank, so no true seeing) and harder to hit, Kairon received most of the Herald's attentions. Even after taking the Servant of Balance path ability to ignore crits extra damage, he didn't survive one particularly vicious full-attack from the fallen angel.
