Character death - how common is it in PFS games

I have seen lots and lots of player deaths over the last 20 months. Some as a player, some as a GM. There's one local player that has died over 10 times on his now level 13 character. I've had three? four? character deaths over my playing career. As a a GM, not counting WE BE GOBLINS (where people love to blow themselves up), I have probably a dozen or so TPKs and PPKs. And then probably 25-30 individual player deaths on top of that.

With the exception of one or two very poor rolls and bad timing, or grossly mismatched fights, all player deaths (mine own included) are the result of poor teamwork.

The best thing any player can do to improve his or her odds of their character surviving a game is to work with their fellow PCs as a team to defeat an encounter. All too often I see characters not holding their action for the bardic performance or haste, attacking without flanking, or walking into a wizard's lightning bolt path. Individually, it's not so bad, but all together, when my dice are hot, it often results in character death.

Adventuring ain't easy, as they say.

I had my first character killed in his third session. After that, I haven't had any PC deaths. Our local VC, on the other hand, recently claimed (IIRC) 14 dead PCs within 4 games...

My current highwater mark for player-team deaths in one scenario is on 4-12 Refuge of Time.

Casualty List:
Dwarf cleric: died once.
Pregen Seoni: died once.
my former VC's barbarian/rogue, Butters: 4 times.
Slippery rogue: none.

Scenario: failed.

Let's see... Then there was God's Market Gamble. BBEG ran out of ammo and fled the scene. Casualties galore.

Then there was the season 0 5-9 adventure.... table badly understrength for their APL was high tier by math but not by output or tactics. "Uhm, sorry to tell you, the [redacted] looks straight at you. Trueseeing. Full attack."

Race for the Runecarved Key: last time I judged it, only one casualty, to con-drain poison.

Revenge of the Rene pt 2:

The vicious circle of death continues as my Pathfinder lost his life in the Forgotten Laboratory of Thornkeep...

Actually, he didn't kill me per se as it was errant alchemist fire mis-thrown by a wizard from 30' away that struck my L2 alchemist who was already inconscious at -9.
