Concerning consumable thrown weapons, I would like the system to keep track of how many you throw, how many hit and how many miss. The items do not have to exist as stand alone items at the end...the last man standing (or perhaps only the thrower, if they are the last man standing), gets a certain percent chance to recover each...and the items show up like "loot" in a window.

Different weapons would have different chances of recovery, for instance a javelin would be easier to find after the fight than a throwing knife. I could even see item modification consumables such as "Red Silk Band, attachable to any thrown weapon." This item would decrease your hit chance slightly but greatly increase the chance of recovery. However, the steel throwing knife would have a higher durability than the wood javelin...meaning the chance of non-recovery due to breaking for the knife would be less.

I would like to see spears have the reach they deserve, along with benefits for and against cavalry.

I love spears though. =)

Everyone likely expects this from me, but, Dark Souls did spears right.

As for the melee/throwing thing, couldn't there just be different attack icons for different uses of the weapon? If you use the 'throw' attack, both go into some kind of cooldown, to represent the item not being in your possession anymore. After that, it's assumed you recover what you can. If you had multiples of the same type of item, then you can use the throwing attack more times before the cooldown starts. If the game has item damage, then I suppose there should be some chance that a thrown weapon is lost, with the odds dependent upon the relative size and fragility of the weapon. Missile weapon ammo could be handled the same way.
