While there's nothing to stop you from holding a weapon in each hand, you need the two-weapon fighting feats to gain any extra attacks, right?

So, how about this situation. I have weapon A in my right hand and weapon B in my left. On my first turn, I attack with weapon A. On my next turn, I use weapon B instead. How does this work? If there is a penalty for attacking with an off-hand weapon, how do you switch them?

Yes.

It works just like you said. You only incur TWF penalties if you take the extra attack in the round with the off-hand weapon. Nothing stopping you from using iterative attacks to alternate between weapons.

There is no off-hand.

No penalties on either attack.

You can get the extra attack without the feat, you just eat a bigger penalty.

Just want to clarify you can attack while holding two long swords and switch between weapons during your attack with no penalty as long as you don't take an extra attack?

Example: Level 6 fighter with strength 10.
Long swords +6/+1
First Attack at +6 with the first longsword
Second Attack at +1 with the second longsword

Yes, that's right. See also this thread.

Yup. You can also take an attack with your longsword and then throw your dagger. As long as you're not getting extra attacks out of the deal, you don't take TWF penalties.
