There are several spreadsheets around, on various sites, like the one used by the Pathfinder Society Online Collective, the online group in Australia, and several more.
I have one in Excel format, more-or-less complete.
If you have an Android device, there is a program available for it, called the PFS Scenario Tracker. I think I picked it up from the Amazon App Store, or it might have been the Google Play Store.
It allows for showing which characters you have given credit for a scenario, and tracks player credit and GM credit separately. The only thing it can't do, at present, is track credit for Tier 1 scenarios and modules. It can also keep separate player accounts, if you are sharing it with a spouse or sibling.

If you're looking for something that doesn't require an electronic device, there is a great checklist on the Southern Ontario Pathfinder Lodge website.
Here is a link to their download page, it's the first document under the "Player Documents" section.

@Kinevon, I don't have an Android smartphone but that app looks like it would be perfect.
@Myron, I looked at the PDFs you linked and I think something like this: Scenario Checklist Booklet is what I was looking for, albeit only for Seasons 0-3 so far.
Something like that should be added to the Guide, but in a more condensed layout.

I'm in the process of adding a section for Season 4 scenarios to the Booklet on the Ontario Pathfinders website; there will be a whole new booklet for seasons 0-4, as well as a single-page Season 4 list to print out and add to the old one. Should have it done by next weekend, and I'll be sure to post here once it's updated.

Here is something I have been working on, is has a single sheet for each season and one for the Sanctioned Modules. I put in lines with blank space for the Season 4 Scenarios that have not been announced yet. And will update the file after new Scenarios/Modules are added.
Wait, you can replay the same scenario/module with different characters? I thought that was only available for a set few level 1 ones?
Or can you replay a module at a different tier with a different character as your Google Docs sheet allows for.
I'm confused.

I think it's a bit of a waste of space, but if you want to track which characters you have used each time you replay the session the extra entries are handy for that.
But that's really only useful for one like We Be Goblins and such.
The Google Docs is almost what I'm looking for except for all those extra columns since you can only apply a Scenario/Module to a single character as a player and then again as a GM according to the 4.2 guide on page 21:
You cannot receive more than 1 player Chronicle and 1 GM Chronicle for the same scenario, regardless of how many times you GM or play the scenario. You are free to replay a scenario in order to meet the minimum legal table size (see Chapter 7), but once you have reached that limit, you do not earn any additional rewards beyond having a good time.

I think I just copied and pasted the lines when I was doing the Modules Section. I can see the confusion I will go in and delete the other lines and repost it. That was just a mistake.

Looks great, but I still don't understand why the multiple columns if you can only get credit for playing once as a player and once as a GM (except for Tier 1 and Tier 1-2).
Am I missing understanding something?

You are correct, I have those one there mostly for recording how many time someone has played/GMed the Scenario. I know I like to know when i ran something and at what Tier and this can be a record for it.
I will be working on a Version 2 that is more detailed having the Tiers of Each scenario and then like the Revised modules page, the column for GM and PC.

Here is an updated sheet with a Note side of the page for each season, and I added blank lines to fill the Sanctioned Module Page so people can add more themselves.
I still had to log in to a Google account to see this.
I'll add the request I make every time somebody comes up with one of these - add the Tier to the module/scenario title, so it's useful when you're trying to find a scenario you can play with a known character.
