1. Shouldn't is a better word. I suppose its possible if that is the way the players really wanted it. The goal is to have players fill the kingdom roles they really want, however it is assumed that someone will be the leader, unless you are running kingdom in the background and the PCs aren't the new kingdom's high council at all.
2-3. Absolutely. There are many games described in this forum where the consort/spouse of the ruler in as NPC, and yes their stats do help the kingdom.

I think an NPC could totally be the ruler of your kingdom. Especially since we're talking a monarchy here, I could see a figurehead ruler whose actions are directed by a council (the PCs).
If your PCs go this route (maybe because none of them have high charisma, or none of them want the role), it could lead to some fun roleplay. Even the best figurehead may get his own ideas once in a while and deviate from the script.
