So, if Yzahnum escapes the encounter in the street with the PCs, there is nothing stopping him from granting wishes to the defenders of Castle Korvosa the following day when the PCs attack it, right?
In my game, he managed to get away from the party pretty handily.
Nothing but his own pride and arrogance. Genies are notoriously fickle when it comes to wishes. I doubt that Yzahnum would WANT to grant wishes to just any old chump on the city wall; that kind of servitude would gall him. He does much of what he does for Ileosa because she controls him, but after suffering a humiliating defeat, I suspect that if left to his own devices he'd just plane shift away and wait a few hundred years before showing his face in the area again.
That said, if you do want to start throwing wishes around against the PCs, he's a good way to go about it.
How and why genies grant wishes is actually a big part of the upcoming Legacy of Fire adventure path, in any case... although that won't really help advice genies running amok with wishes in games going on right now, alas.

Before Legacy of Fire comes along and rains in our collective "Grain-ee-ohs", have said wish-granting efreet tagging along with any and all Named Baddies - such as the head of the Red Mantis assassins, the simulaqueen, either of the two Big Bad Ba'atezu (or, best with both of them plus the efreeti "on call") or even doing the "red mantis buzzing about the dragon's head" thing...
