Anyway, I think I remember Tanarukk being about CR 2 or 3 in the Monsters of Faerun book, and they had outsider racial hit dice at that time. I would say that Barb 2 (which is CR 1) plus fiendish, advanced, and SR translates to about a CR of 2 or 3. If you want to up them to about CR 4 I would add another level of barbarian.
Alternatively, you could start with a basic orc barbarian and drop the half-fiend template on them... but that might be a little too much oomph for not enough HP. I'd still probably tack on a level of Barbarian.
What kind of stat array and feats are you looking at, though? That could make all the difference.
With stat mods they are at 38HP, AC19, and 2d4+18 with falchions when raging with PA. They also get the DR5 cold and fire with SR12. I upped their natural armor by 2 and SR to 12. CR2 seems pretty low to me. CR3 is making more sense than last night. Power attack and toughness for feats and a a flaw that drops their speed.

Wait, orcs have natural armour?
Oh, and I just remembered that smite evil persists until the target is dead. And it gives some pretty good bonuses. I think they might work pretty well as CR 4.

Hmmm, that strength is deadly for the lower CR. I would definitely add a barb level and swing it's CR up a point. Alternatively, you could tack on two levels of warrior. You get more accuracy and HP for a lesser CR.

I found out that dropping two NPC levels into a build does little to modify it's challenge (beyond the HP). What level is your party? 2d4+21 is going to be some nasty damage. :)

7th and 8th. My biggest challenge is knowing who will make any given game. We vary between 3 to 8 players so scale-ability is handy.
The damage is respectable especially with a crit, but the party usually has a very good combat healer and may have 2 sorceresses and a wizard on some nights. They can be a challenging group to GM for sometimes.
