I think the primary complaint that everyone is missing with this issue is that what the market was looking for was some degree of guidance and change (Growth as opposed to carnage for the sake of a buck) in the setting. Now Before TSR rolled belly up the Mystaran Setting had developed the 'Poor Wizard's Alamanc' which provided an annual set of events that would shape the setting over time. This is distinctly different from The Use of Adventures to determine the story. because even if your players failed to achieve the goals of the adventure - the setting was progressed acording to the 'agenda' of the company.
Forgotton Realms seemed to mix a little of both, but the setting progressed. Effectivly the people who play the game need greater influence on where the Settings go. Sure there can be some massive horror unleashed by the company in the latest series of campaign...but It needed more player input as a consequence of their actions.
