Some of Ghartoks stats seem off.
To be specific:
His initiative: -1 dexterity, +4 improved initiative, +2 foresighted= +5 instead of +0
His full attack when raged:
+7BAB+10 str-1 size+1 magic= +17/+12 with goreshred instead of +15/+10
Am I right or am I missing something?
He also wields a medium greataxe that has a base damage of 2d6. Is that a mistake of is he supposed to have a large greataxe (which deals 3d6)?

You are right for ghartok's initiave. It's +5.
As for goreshed, my guess is that he got it before his transformation into an unchosen, or otherwise, it was originaly made for a medium creature, so that's why it is a medium sized greataxe. Since he uses a inappropriately sized weapon, he suffers a -2 penalty on his attack rolls.
If you are using Pathfinder RPG rules, you may want to have a look at these conversion documents on The Archives of Nethys. Most of the statblock mistakes (like Ghartok's initiative) have been corrected.
I am still confused. Why is its damage 2d6? It should be 1d12 right?
Also I thought you only took the -2 penalty for wielding weapons fitting for creatures of greater size than you, not of smaller size than you...
EDIT: Just checked it in the srd, yes he should take another -2 penalty so his greataxe full attack correctly is +15/+10.
