Zin misses the remaining Mi-Go!
The Mi-Go, for its part remembers Oh yeah, my race can fly. Those other guys sure were dumb huh? and skedaddles to hang out on the ceiling in the corner. Withdraw. It watches the table intently.
As it does so, the man on the table starts screaming. But only for a moment.
Another tentacle bursts from the man's head, just like the priest from before!
While strapped to the table, it still lashes out at the nearest target: Jones!

Constrict Damage: 1d8 + 4 ⇒ (5) + 4 = 9
Its touch begins to drain Jones' psyche.

Darius kills the last Mi-Go and harms the Spawning Canker.
Then all Hell breaks loose. Claxons and flashing lights sound around the room, the depictions of space on the ceiling replaced by alien texts in red and yellow. Though none of you speak Mi-Go, the meaning is apparent. WARNING! DANGER! WARNING!
The Canker, in turn, explodes as the man's head did, the torso ripping asunder and sending chunks flying as a great, tree-like hand erupts from the body, followed by another.
The head follows, branched and festooned with a diseased moss, uncountable mouths appearing and disappearing at random, dripping vile fluids. More and more bulk pours from the remains of what was once a man, too much for such a small form to contain, until the abomination looms over the room, tentacles waving and maws gnashing.
This is an entirely different creature, so inserting a new Initiative. It is Huge sized, and Kn. Dungeoneering applies.

Whoa wait a sec, did this creature suddenly spawn out and attack me after spawning out and doing that.. again? Because then it basically got a free attack in between people's turns. Either way Im likely dead If that is the case.. SUDDENLY BOSS Fight monster does that when it gets 4 +16 attacks that do +16 damage each.

Sort of. This was really bad luck. I rolled Initiative for the Spawning Canker when I rolled for the Mi-Go, and it got a 7 (so, dead last). I kept it off the board since it wasn't really visible as an enemy until its turn, at which point if exploded out and attacked Jones. It was then your turn, and damaging it triggered the appearance of the Dark Young (an entirely separate creature, and the REAL boss fight) who happened to roll an Initiative score that put it right after you.
However, it would stop attacking once you're unconscious, since it, well...it likes its food eaten slowly, while alive.
The first attack + Constrict did 34 damage. Did that put you under? The second attack would then do another 18. If the 34 puts you down, that would be redirected to 1d2 ⇒ 1 Jones.

Depends, How many rounds went on? Because precedent wise this was as if having 3 or so readied actions go off at once. You had first confused me with the Sudden Darius was up after I went. Then seemed to ignore it. Because of this Im unconcious I think from the combo Im just also probably dead.... because of those Guns.. I hope they Sell for 20k+ each

Casting Terrible remorse on it. Every round for 11 rounds he makes a will save DC 25. Even if he makes the save he is staggered and has -2 ac. If he fails, he hits himself for 1d8+Str mod and that's his turn.

Darius's eyes widen as he strikes down the enemy with impunity but spawns an abomination. As the tentacles reach out and try to squeeze him, Darius dances Grimoire in between then strikes. Two get in regardless of his efforts and crush his bones and whip against his body, tearing into his flesh from the strangulation. As he grins, struggling to get out, eventually the life comes from his eyes and a resounding snap signals his form giving out. The snide grin remained on his muzzle for but a moment after.

Sledge will probably finish it - Zin, save your spells, this thing probably has a fair amount of resistances and we'll need healing pretty darn badly.

With two swift attacks, Sledge tears the beast asunder, it collapsing under its own weight.
Everything is silent, for a moment. Then just as startling as it appears, the Dark Young is sucked back into the bloody remains of the man on the table, body reconstituting in reverse as it does so. Soon the Spawning Canker reappears, and the man's head reassembles as he let loose a strange backwards scream.
A rift in space-time crashes open, through which all of you catch a glimpse of the true form of Shub-Niggurath, just for a moment. Roiling masses of hair and flesh make up the bulk of its body, interspersed with massive tentacles, hooves, fins and mouths.
The most horrifying are its thousands of eyes, each bearing a soulless gaze, seeming to be portals to infinity, showing the infinite blackness of space and stars beyond.
It looks at you, and it doesn't even have to attack your mind for you to feel the effects of madness creeping in this time.
The man's body is drawn into the portal,screaming in terror once again...forward this time, conscious as he is flung into Shub-Niggurath's waiting embrace.
And then it closes, sending a shockwave around the room. The great dome holding back the water cracks, and water begins pouring in. Not gushing, yet, but filling steadily.

Issue is I think you lose the mental Bonus as thats a Racial Bonus. Along with needing WISH or Miracle to get me back to my old body. I dont think we know anyone with that magic. An Lol, Darius would be young again.

I think you keep mental bonuses - the language ("Strength, Dexterity, and Constitution scores depend partly on the new body. First eliminate the subject's racial adjustments (since it is no longer necessarily of his previous race) and then apply the adjustments found below to its remaining ability scores.") seems to imply that the only racial adjustments removed are the physical ones. If you don't gain the mental adjustments of the new race (which you don't), that indicates your mind is unchanged - so you keep what you've got.

"If anything the idiot is probably unconscious, I'll heal him."
Z approaches the body of Darius and channels her magic into his body.

Darius's form doesnt move or respond, though he was still warm to the touch. There however was no sign of breath nor beating. He did not even snap at Zin as he normally would. He was gone. The Spell would not disperse.

"I'm just starting to get pretty pissed off at his antics!" Z says shoving the corpse of Darius.
"We have people who are legitimately injured and he is playing games!" Z looks around the room, her blank expression and glassy gaze not matching the anger in her voice.
"You're not fooling anyone!" she yells down at Darius, her flames growing more excited.
