Stupid Reach Question I Should Know The Answer To

Rules Questions

If a creature has reach, does it get an AoO every round or only the first round of combat before its opponent "gets inside" its reach? I'm having a bad mental health day and things are not computing and I can't focus enough to find the answer.

Thanks in advance.

I believe AoO are based on enemies moving out of a threaten space. Not really the round

That's correct, though if you start getting into the 5 foot step shuffle, characters rarely provoke after walking into melee with reach characters.

It only gets an AoO when a relevant action provokes. The turn it is on has little to do with it, unless your reach critter is flat-footed.
It is entirely possible to run through a creatures reach without provoking at all if you win initiative and the big beasty doesn't have combat reflexes.

Thanks, ya'll. I'm currently working on an adventure I'm calling "The Black Psalms of Azathoth" and a Bronze Giant is one of the obstacles the players will need to overcome.
