How to add offhand attack to AoO

Advice

I know that I've seen at least one mechanic that allows a dual-wielding character to take both a primary and an offhand attack when they get attacks of opportunity, but I can't find it (them?) now.

Iirc that was a feat that did allow that.

AFAIK, the only rules element that lets you do that is in the Two-Weapon Warrior archetype for Fighter. Equal Opportunity allows you to make an AoO with both a main-hand and off-hand attack with the appropriate TWF penalties.

That's probably what I'm thinking of Kazaan. Too bad, since the two weapon warrior doesn't qualify for advanced weapon trainings.

I seem to recall a 3.5 Feat that allowed that.

Double Hit, a Combat Feat--I believe they used to be called Fighter Feats--from D&D Minis, page 25,

I found it on Crystal Keep.

Yeah, and they aren't really even all that great at TWF. Mobile Fighters do it better since they get to move around while doing it, Weapon Master Fighters do it better since they get early weapon training and 99% of TWFers use the same weapon anyway, Rangers do it better since they get Style feats that ignore prereqs, and UnBarbs do it better since their Rage is a flat bonus to attacks rather than a Str bonus.

off hand opportunity is the name

That's giving an extra AoO, that specifically uses the offhand weapon. Not a bad benefit, but not the equivalent of the Two Weapon Warrior's ability either.

Pretty sure that just gives you another AoO.

This probably isn't exactly what you meant, but the fortuitous enhancement is kinda similar.

Fortuitous basically gives you an "iterative" on your AoO; a second attack at -5.

Improved Two-Weapon Fighting Stamina Trick allows for a 2nd AoO

Oooh. Two weapon combat trick/fighter archetype with a pair of fortuitous weapons, possibly with a level of swashbuckler to Opportune Parry and Riposte. You attack me, I parry and swing back four times.
