What makes a table run well is a GM who has a firm grasp of (most of) the rules who is intent on making sure everyone has a good time.

What makes a table run exceptionally well is for that GM to sit at a table full of players with that same attitude.

In my 160ish sessions of PFS (that's player and GM) Ive had a few crummy ones because people were too involved in making sure that they had a good time themselves and that things went according to how they should to do anything towards making sure that everyone had a good time.
