NPC gear and treasure

Rules Questions

I'm rolling up some random encounters, and when I get to one that lists treasure as "Gear and Other Treasure" what does it mean I assign?

1.) Just the listed gear?
2.) Just the listed gear and extra stuff to give it equipment equal to a npc of its CR based on the NPC equipment table?
3.) The listed gear and treasure for a monster (or group of monsters) of its encounter level?
4. Listed Gear up to NPC value on the table, and the monster treasure drop indicated by its encounter level?

The way I run it is if it says, "Treasure - Equipment, Combat Gear" then I always roll up an appropriate gear/treasure loot list of the encounter, and stow away the results around the area, on certain NPCs, hidden, or otherwise.

If it read "Treasure - Combat Gear" then you simply have them lie with what the came equipped with, maybe a few stashed copper-to-platinum coins, generate some nonsense loot like 3 sharpening stones, and a cracked smoking pipe. The player eat those smoking pipes up though, like candy.

Just the listed gear mostly. The GM choose the "other treasure". That other gear is often mundane equipment. I don't often bother with it though since most of my players just care about the bigger items.

If the NPC is a humanoid they have a chart for how much gear they should have.

Monsters with no class levels have a different amount of wealth even, if they are the same CR.

I'm thinking of fudging most of the smaller things, even stuff like potions and the like. Still, it would be nice to not have to fudge it and just slap on some general gear per level for npcs, perhaps.
