It may be a Reader or LibreOffice issue.
I am using Foxit 5.3.x and Open Office 3.4.0 and the issue does not exist, ie. all the carriage returns are normal.

My issues are from Stat Blocks, the first two acted this way for AP 61.
And since it works for AP 60 I can't see it being a reader problem.
And since it doens't work for Wordpad I can't see it being a LibreOffice Problem.

It could be something to do with the styles or how the text was formatted when it got put into InDesign. I could also be totally wrong and it's something to do with how we ended up styling it. We also recently updated our word processing software in house, so it *could* be something to do with that. Do you mind sending a screenshot to chris.lambertz@paizo.com? I'll see what I can find.

Chris, whilst you're investigating, my one file per chapter version of this is having the same graphical weirdness the players guide was having.
I am using the latest version of Goodreader on an original iPad

Still works for me. Here is a LINK to a .odt file that has a stat block from each of the two pdfs.

There never is. Foxit always copies it as "plain" text

So, after testing this in our office between both OpenOffice, Libre Office, Microsoft Office and Text Edit, the best answer I can come up with is that after our update to the newest version Office, there were changes in how it's handled and funneled by InDesign and in turn, Acrobat/Reader. For instance, if you paste as Styled Text in Office from Adobe Acrobat/Reader, you will get the text as close as it appears in the PDF file (the other applications retain everything but the carriage returns). If you use an alternate PDF viewer such as FoxIt or Preview, when you paste as formatted text, it retains the styles and carriage returns. I suspect this is some sort of bug with Acrobat/Reader, and wonder if a future update will fix this. For now, there's not really anything we can do in the office to fix this issue.
