Planar Ooze
So vast that it itself is an entire plane of existence.
It generally can only be interacted with by plane-shifting to within the Ooze itself,
where it controls most of the basic qualities of existence.
Most interactions of this sort end with complete assimilation, though brave adventurers may dare fate.
... :-)
Reminds me of the elemental plane of flesh.

I really liked the Leshay from the 3.5 Epic level handbook.
Of course I never saw or played one (to my knowledge), but I like the concept.
I used them as master manipulators from an alternate dimension that made sure a prophecy came true concerning the party (prophecy involved the resurrection of their god).

Okay. These are in no particular order.
1. Epic templates. Things like the Paragon and Pseudonatural templates from the 3.5e SRD. Not necessarily those specifically; I just mean templates that are suitable for making a non-epic creature epic.
2. Above-Colossal creatures (with associated rules). There was a nice set of above-Colossal rules in the Immortals Handbook Epic Bestiary.
3. A demon lord "toolbox." This is different than a template; I'm thinking of the awesome article from the last issue of the Dragon, where there's a set of guidelines for creating epic powers. Heck, don't restrict it to demon lords - this toolkit is how I see the epic inevitables/demons/devils/proteans/qlippoths/etc. being created. Like James said, they should be individual creatures.
4. Epic elementals. The primals from the 3.5e ELH were okay, but really didn't have the presence I was looking for.
5. Epic CR Lovecraftian beasties. These should strike fear into even the planar powers.
6. Big swarms. I'm tired of all my swarms being simply lots of 10x10 swarms next to each other.
7. From the ELH - these are the creatures I like and want to see brought forward: Atropal, Behemoth, Colossus, Demilich, Devastation Vermin, Dragon (Advanced), Genius Loci, Gibbering Orb, Gloom, Lavawight, Legendary Animal, Living Vault, Ruin Swarm, Shape of Fire, Thorciasid, Umbral Blot (Blackball), Uvuudaum, Winterwight, Worm that Walks ... though some of them could use some cleanup.
8. Epic golem rules. The Immortals Handbook Epic Bestiary had some general golem rules, but they were hard to use and resulted in creatures that were overly hard for their CR.
9. I'd like to see the leShay fixed. I'm 100% positive that the leShay in the ELH were changed at the last minute; my guess is that originally they had something like 40 levels in whatever class they liked at the time and someone took that rule out - after all, what else would this rule for their weapons mean?
LeShay Weapons (Su): Each leShay carries two personal melee weapons that she manifests from her own life essence. The weapon can change form as she shifts from class to class, typically appearing as a bastard sword (one in each hand).
Switching from class to class? Huh? That would probably explain the inexplicable CR50 for the thing.
10. Epic undead. The Dicefreaks folks did a great job with this, with epic templates for ghosts, death knights, liches, etc. Something along those lines would be great.
11. Epic challenges. Kind of like how haunts aren't technically creatures, I'd like to see more of the same. The ELH had flux slime (or whatever it was called), but that was just silly.
12. Rules for awakened/intelligent versions of strange things. What would an intelligent castle be like? Or an awakened mountain? Or a living demiplane?
In reality, lots of what would be in an epic bestiary would be as much rules as cretures - I would hope the whole thing would be a lot more of a toolbox than the ELH.

Yeah! I also had the leShay as behind the scenes manipulators, except there were only two of them and the leShay did the Trading Places thing, except with the character's whole home realm at stake. The players were SO pissed at the end when they encountered both of the leShay at the end and the players realized what had happened. One of the leShay was currently a vampire just to see what it felt like, and they had an epic inevitable hanging around to make sure they honored the deal.

Pretty much most of what's listed here and hasn't been covered, along with Colossal+ rules.
I hope Paizo can do better than these. Many of these were not impressive or very imaginative. There are a few nice things in here, but yikes, I expect much more from Paizo.
I'd love to see epic rules and some very epic monsters, but please let's do better than reving up most of the monsters we have already. In other words, I'd love to see an epic bestiary, but not if it is just a bunch of mega - undead, golems, elementals, etc, that also lack any unique flavor.
