Name: Bobarella, Freddy, Larry
Race: Elf, Elf, Human
Classes/levels: Barbarian 5, Fighter 5, Alchemist 5
Adventure: Carrion Hill
Location: Rupman's Vat
Catalyst: Spawn of Yog-Sothoth
The Gory Details: (optional) While the followers six were thoroughly successful in slaughtering everything else with alacrity, they got their butts handed to them by the Spawn even with its 6 negative levels. Bobarella was eaten first, then the Fighter (at which point the Rogue fled through the big hole in the wall, nearly eaten by the provoked attack of opportunity). At this point, the Wizard - while on fire after being helped out of the middenstone vats by the Cleric - used the teleport within the Pnakotic Manuscripts and whisked themselves to the safe proximity of their Boss Characters.

During the few rounds' time that the rogue was bok-bok-buckawing away from the Thing Man Was Not Meant to Know, the alchemist had also slip-n-slided into a burning cat of middengoo (that he himself had previously ignited in order to incinerate some zombies). Itself impervious to fire, the Spawn ate the alchemist as he did the other two: alive and screaming.

Teach them to let themselves be too spread out inside the warehouse. ^_^

Name: Harold
Race: Human
Classes/levels: Mobile Fighter 5
Adventure: Golden city of Death
Location: Isle of Terror
Catalyst: Golden guardian
The Gory Details: After being knovked out by the golden guardian, team ranger shot the thing dead. Als, they did not see this coming: Thing melted to the pool of molten gold, and covered Harold, killing him.

Name: Fanta Fizzlepots
Race: Lava Gnome
Class/Level: Alchemist 4 (chirurgeon)
Adventure: (part 2/3 Crypt of the Everflame trilogy)
Location: main temple of Razmir, main stairs
Catalyst: foolishness, mayhem and pyromaniacal violence!!

Gory Details: We had gotten our goodies, buttoned up the mooks in the dead of night and CDG'd the big dumb acolyte of Razmir guy before making our way upstairs to where 2 peons guarded the upper stories from nosy do-gooders bent on killing the bad guys, taking their stuff and burning this wretched temple to ash (us). Guess you could say that we were going postal on Razmiran tucas.

Needless to say a royal slobberknocker ensued as the disgruntled ex-cultists (us) took our umbrage out on the sorry bastards fleecing, extorting and intimidating our new home town's populace without cutting US in on the action!

At one point we are hip deep in flank-happy kidney skewering cultist rogue types and the damage was really piling up. Single target attacks are not doing the job fast enough so out come the unstable incendiaries en masse. A series of rounds take place wherein several rogues get firebombed, I get stabbed and fall down, the cleric riffs off an awesome channel, firebombs fly, rogues continue doing the Tiberium candle dance, etc. After 2 or 3 rounds of this the rogues finally wise up and eviscerate my beloved gnome beyond the ability of a channel to bring me back.

We win, the temple is razed and a Reincarnate is dropped on Fanta, coming back as a fire suli.

Which leads to ...

Class/Level: Alchemist 5 (chirurgeon)
Race: reincarnated fire suli (bestiary 3 playable race - don't ask, the GM determined the race)
Adventure : City of Golden Death
Location: somewhere that a babau demon pops up and does nasty things

Gory Details: Since time was decidedly an issue, Fanta secured a scroll with a Reincarnate and a Restoration (including both 1k gp M components) from Kessel before making the journey across Lake Encarthan onto the Isle of Terror. Fanta had gotten spiffy Medium lamellar cuirass armor, swapped out his old Small musket for a new mwk Medium musket, a handy haversack and a pile of powder horns and assorted regular, cold iron and silver bullets.

We'd commandeered the free ship, put the dwarf crew in charge and cleaned house en route to the namesake city. We kicked statue butt, we kicked elemental butt and scored some swag.

Sadly, Fanta was summarily dispatched by an unarmed babau demon that caught Fanta completely off guard and lacking all buffs. A flat footed AC of 15 didn't even phase the demon's three sneak attack natural weapons. 53 damage against 28 hit points at full (down 5 due to a lingering permanent negative level) put the poor alchemist once more in the dead house at -25 hp.

Those who know will recall the spectacularly unpleasant flavor text regarding the Isle of Terror. Fanta is once again a lava gnome - with the terror creature template and now 3 permanent negative levels!

A NE pyromaniacal gnome that is immune to fear, absorbs negative energy and has a fear aura ... if the poor bugger survives this wretched place, he is going to need some serious therapy.

Since there's a lot of modules, and most people have likely not experienced ALL of them, it might be good to keep all of the obituary details in a spoiler tag. That way, if you've played/ran a particular module, you know it's safe to read.

Just a suggestion.

Internet ate my post. I've flagged my post for needing spoiler tags - thanks for the suggestion!
