Since you have already converted your issues, all you need to do is purchase a Month-to-month Pathfinder subscription through the normal means (i.e. put it in your cart, go through the checkout process, etc.).

Your order will create an authorization on the credit card at the time of checkout, which is a non-monetary transaction that ensures the card is valid and has sufficient funds, but no money will be taken at that time (however, many banks will place a hold on funds based on an authorization, but these will expire on their own). The first charge will come the month of your first month-to-month volume, which is after all of your transferred volumes have been sent. In your case, this will be Pathfinder #3.

Issue #147 is, indeed, the final issue of your subscription. To get the final two issues of Dungeon, I'd suggest you renew with a Month-to-month subscription. And then, of course, roll it over into a Pathfinder subscription.

Thanks!
