WEIRD SCIENCE-FANTASY HORROR SETTING
Carcosa is a science-fantasy role-playing game setting with a sandbox approach: whether heroic or opportune, adventurers of all stripes will find freedom and consequence worth their mettle in a horrifying milieu mixing adventure fantasy, the Mythos, and comic book sci-fi with no punches pulled.
Carcosa is compatible with Lamentations of the Flame Princess Weird Fantasy Role-Playing Game and other old school fantasy adventure games. This expanded edition details 800 encounters on the 400-hex map and includes the starter adventure "Fungoid Gardens of the Bone Sorcerer."
The included PDF version of Carcosa is fully and extensively layered, bookmarked, and linked—truly a state of the art piece of PDF technology!
Warning: For Adults Only! Contains explicit descriptions and illustrations of vile black magic and violence.
I asked Fitz from Game Knight Reviews to sit down with Geof, the author of Carcosa, on tape. The first part of their discussion focused on who Geof is, etc.
I think it's better to read the first part of James Raggi's review (which, admittedly, if mentioned in the article Skeld linked to) to get not only an overview of the controversy (and the first version of the product, which is not the same as the version above), but also for what I consider to be an excellent editorial on the controversy itself.

Wait .. what? The PDF is censored? To heck with that, gimmie the unexpurgiated version. If I don't like it, let me decide. If need be, PM where I can get the unexpurgiated version.
Me too. Definite thumbs-up to the guy for his bravery in standing up for something so controversial.

The PDF here, in Paizo's store, is not censored. The links in question are about the original version of Carcosa, which was a little brown booklet sold by McKinney directly, and had both expurgated and unexpurgated versions. The new version by Lamentations of the Flame Princess, on sale here with expanded content from the original, has only an unexpurgated printing.
The PDF version is flagged as adult content, and the print version is what is available through distribution. I've flagged the print version as adult content as well until I've got it clarified.
