I, too, seem to have become a victim of the great Missing Gazetteer monster.
Also of note: while I've received Pathfinder #9, the orders page still says "shipping". And there was no packing slip.

Interesting...the exact same thing happened to me. I've actually had two orders that were placed and shipped after my order for the Gazetteer (which apparently shipped on the 17th), and all have arrived in the mail as of today, but still no Gazetteer.

Steven,
I apologize for the missing book! I will get a Gazetteer shipped out to you right away.

Donald,
Actually, it's not really the same thing. Steve's (and others') subscription shipment which should have contained the Gazetteer and Pathfinder #9 showed up with just the Pathfinder inside.
Your Gazetteer shipped out to you by itself on 4/25 via USPS Standard Mail, which has an estimated delivery time of 3 to 10 business days. Today marks the sixth business day since it was shipped, so it should be showing up at any time now. I know it seems counter-intuitive, but it actually isn't unheard-of for one package to "pass" another in transit. This has no bearing on the shipping time of the previously-shipped package.
Monday update: I just got an email telling me that another copy of the Gazetteer has been placed, for $0. The prior order *still* says "shipping" on the order summary page.
I don't know if this is some sort of automated magic "fix it" system, or if Cosmo is working his magic... But Thanks!

Looks like I'm in this boat, too. My order (#935583) was supposed to have PF #9 plus the Gazetteer, but last week when it arrived, it had only PF #9. If you're still reading this thread Cosmo, could I get my copy sent? If I don't see you respond to this, I'll start a new thread. Thanks!

Same here. The order (#945413) just got here today and I got Pathfinder #9 but no Gazetteer. I just sent an email to customer.service@paizo.com, but figured I'd check here too.
Add me to the list. Order #935615. I got the Pathfinder #9 today but no gazetteer inside. Cosmo, I would like to add that I am willing to wait on shipping time if you want to wrap it into the next monthly order.

My bad. Understand that shipping can be quirky and is completely out of Paizo's control. Thanks for the explanation and I'm really looking forward to getting my Gazetteer!

I sent an e-mail yesterday but haven’t heard back, so I just thought I’d post here as well.
I received Pathfinder #9 but my Gazetteer was missing. (Order #935438)
I thought you had that goblin problem under control...if you need me to swing by and crack some goblin skulls you know how to reach me.

I am missing the Gazeteer from my order (#935202) as well.
Same here (order # 943682). I received the latest Pathfinder adventure but was missing my gazetteer. I sent Paizo an email and left a voicemail with customer service (as they were busy feeding the kobolds)
Your missing copies of the Gazetteer are on their way.
As for the question of: "What happened?"
There wasn't anyone other than the usual Warehouse Monkeys packing subscription shipments this time (Sorry, Kruelaid... No hobos). As you may or may not already know, for a big subscription shipment we prepack envelopes with the various package permutations (Pathfinder only, Pathfinder + Module, Pathfinder + Chronicles, etc). Then we print out mailing labels and affix them to the appropriate package (you may notice the product # of each of the contents of your subscription order on your mailing label). This is why subscription shipments do not usually have a packing slip enclosed.
To the best of our determination, a stack of "Pathfinder-only" envelopes got mixed in with the "Pathfinder + Gazetteer" envelopes. There really isn't a big weight difference between the two, especially when one is focused on getting the label on and the shipment out the door as quickly as possible.
Rest assured that the same Monkeys who were probably the mis-labelers, are also the now-chastened Monkeys who get to deal with this extra glut of replacement Gazetteers. :)
So I, once again, apologize to all and sundry for the missing book and the inconvenience. We have and will learn from this incident to ensure that things like this will not happen again.
Thanks,
cos

Add me to the list of subscribers who did not get the Gaz with PF 9.

another one missing the gazeteer from their order here...
that must have been a rather big pile.... any people with PF9 get both on accident?

If so, they're not going to admit it!
While I'm worried I'll fall victim to the same (my deliveries always seem to come after most other subscribers for whatever reason), I am sad to see Paizo have to cover all the additional shipping cost and possibly lose stock of the Gaz if people got it accidentally and are dishonest about it. This is why I'm hoping to get it in with PF9, or if not, include it with another, larger order I've already placed to save Paizo on shipping.
My order is missing the Gazetteer also. Order #935708.
I'm fine with just sticking it in with my next order.

I will get a replacement put in with your next subscription shipment.
Thanks,
cos

Alas, I am also a victim of the mail room gremlins. No gazetteer(order #935691). I am about to place an order so if you want to just add it to the order or include it with my next shipment.

I will put your book in with your next subscription shipment.
Given the increasing scope of this mishap, I am going to be bundling the reshipments with future subscription shipments as much as possible from now on. I hope that this (and the fact that you do already have access to the PDF) will be sufficient for everyone.
I am also missing the Pathfinder Gazetteer from my order(#935358).
If you are willing to put up with the hassle, you can put it in with the order I'll be placing Friday(pay-day). If not, then just send it at your earliest convenience.
Crazy. I'm in the same boat. I was very confused for a couple of days since I thought it must have come before Pathfinder based on when I got confirmation. But I'm not crazy or confused - just unlucky. Must have been a big stack of envelopes.
Please add me to the list and thanks in advance for taking care of it.

Cosmo,
My order (#943478) just arrived and is also missing my print copy of the Gazetteer.
Thanks for the explanation and the quick fix.

We need "The Warehouse Monkeys Ate My Gazetteer" t-shirts.
Add me to the list. Order #947871 has arrived without the Gazetteer.
I've downloaded the Gazetteer PDF, so I'm content for now and in no big hurry. I have no problem with you sending the Gazetteer with the next scheduled shipment.
I hope none of the PF9/Gazetteer prepacks got mixed in with the PF9 only prepacks... Being short on Gazetteers and unable to fulfill existing orders would turn this into quite a Charlie Foxtrot. I wish you luck.
Just got home from a long week of work in Atlanta and was excited to see my Paizo package on the counter when I got home... But NO Gaz... Order #935484
