Only if you are willing to provoke an AoO by skimming the fog, or go totally into the fog, which is opaque and will then render you effectively blind until you come out.

What fog?
The entire ceiling is covered in a steam/fog that wafts up from the lava. Fek used an arrow to estimate it is 4-5 feet thick before you reach the ceiling.

Rhuul keeps up the quips.
"I really appreciated that Ravine, keeping the Juggeraught on the other side. It is almost lavable..."
As he does so, shining light appears at his feet, spreading out to those nearby that have been injured by the lava bomb.

Sorry, I missed that post. The mechanic is just to have clear line of sight to it (marked on the token) and the ability to attack. The effect of aiming for the exposed area is that you can choose either a +4 to hit or to ignore the Juggernaut's DR. I'm assuming you'd take the "ignore DR" option since you're not using adamantine.
Because the Juggernaut is flat-footed, sneak attack damage applies.
Fek takes careful aim. Inspired by Rhuul's levity and the sight of his banner, Fek find his aim to be true. The small missile goes directly into the exposed area!
However, before your eyes, the cracks formed by Narika's cold blast begin to seal up. The rumblings of the magical-mechanical engine fill the room as it starts to move. It rolls across the chamber to the small bridge leading to the puzzle chamber. Its wheels spin as it hits the greased bridge, slowing it down. It then reaches the disturbed tiles, and crushes them as it slowly grinds across.
Double move through some rather inhospitable terrain. You also observed it heal some of the damage it previous took.

Sihir launches a very pretty but sadly not very effective firework at the juggernaut. The skeletal driver appears to be laughing, but it's hard to tell as it has to concentrate on the rough ground it is traversing.

Narayan swoops in, skimming the cloud near the ceiling as he reintroduces Fossilblight to the wound it had caused!
Since if Fossilblight left the wound it can probably punch through the DR, Narayan will take the +4 to hit

Narayan, did you mean that you are going into the cloud, or staying just below it? You have enough movement to get in front of the juggernaut without provoking, but if you are trying to surprise it by coming out of the cloud, let me know.

Rhuul's humour really gets under Tiny's skin (and Narayan, and one other if they want it). Anger bursts forth as his muscles pump up and he feels the only way to release it is to hit something. preferably not the bard
Rage spell, Tiny, Narayan and any one other person who would like it. +2 to STR and CON, +1 to Will, and -2 to AC, no worry about fatigue at the end.

Rhuul's jokes continue to both enliven and enrage his comrades, who want to end this combat just to make them stop.
Narayan skims the cloud layer and moves around the juggernaut, coming in front of it to strike its weak spot. He strikes true, and notes just how very sharp Fossilblight is. I'll go ahead and tell you now: it is a +3 Keen adamantine heavy pick. Adding in the +3 to hit and damage to your roll, plus the bonus from Rage.
Taking advantage of the juggernaut's distraction with the terrain and the sudden arrival of Narayan, Fek darts from bridge post to bridge post, waiting for an opportune moment to strike.
With the dislodged tiles slowing it and Narayan too high to get under its wheels, the Juggernaut kicks up and tries to slam Narayan out of the air.
Slam: 1d20 + 24 ⇒ (8) + 24 = 32 damage: 4d6 + 19 ⇒ (2, 6, 3, 4) + 19 = 34
Even buffed up, that hits Narayan.
As before, you note damaged sections of the beast are repairing themselves before your eyes.

Sihir calls the shadows together into the the shining form of a lantern archon. The illusory outsider fires a ray of light at the juggernaut, but doesn't quite seem to have its aim down.

With a deep rumble, the shores of the lava river overflow, and lava begins advancing northward toward the main group. As it does, the room shakes again, knocking people to their feet as more lava bubbles fly through the air.
No one got engulfed by rising lava, so that's good. Once again, I need one DC 15 Reflex save from everyone on the ground to stay on your feet, and then another DC 15 Reflex save to reduce damage from the flying lava bombs. A successful save reduces damage to half, and evasion does apply. Sadly, shadowform does not protect against objects and environmental hazards.

Actually, there should be a third point of healing from this round as well. Though not for Narika as I think she missed out on the first round. As Rhuul tells his jokes the glowing path extends away from him as if trying to get away.

Fek slowly approaches the juggernaut, keeping to cover to take advantage of his preferred mode of attack.
Rhuul continues cracking wise, causing his own wounds to heal even as he inspires his comrades to acts of violence.

As Tiny moves into position, Jing Kwan tosses off another handful of sparkly dust, making the juggernaut even brighter, but still not managing to get into the skeletal driver's eyes.

Ignoring the flying lava, Narika throws another ball of snow at the creature; the combined assault is wearing at it, and it is clearly affected by the snow, though not as much as she had hoped.
Finally, Narayan becomes a blur of fists, feet, and tail as he whirls around in the air, repeatedly striking the exposed area of the juggernaut. The combined effect of the snow and the monk's blows are too much for it, and the juggernaut finally collapses.
You pause for just a moment to make sure it really has stopped. As you are about to breathe a sigh of relief and congratulate each other, the chamber begins to rumble again. Within the shaking of the room, you can make out Old Man Mountain's voice:
I WILL NOT BE ABLE TO CONTAIN IT MUCH LONGER ... DO NOT TARRY, OR YOUR FATE IS SEALED.

You run and fly your way back out of the caverns to the surface, the lava just barely behind you the entire way. Remembering Old Man Mountain's promise to divert the lava toward the former site of the shrine, you run down the opposite face to get as far as possible from the devastation.
Not far behind you, the lava emerges, and you see it flow in the promised direction. If you had known about the ancient spirit, you would be tempted to think it was all a natural process and chalk it up to good luck that the lava spared both the village and the ruins, but you see the hand of the the Old Man in the subtle channels that spring up in just the right places.
Beyond that, you see no sign of the spirit, and begin making your way back down through Akilhi's stairs. Once outside of the tori gate, you find Old Pak watching the plume of smoke and ash rising up from Toam Puong. "Well, that's exciting and beautiful ... though I suspect from your point of view it was probably more the former than the latter. Come, come inside before the ash starts to land. We'll wait it out in the Broken Temple."
He leads you back to the temple, and offers you refreshment and a chance to clean up, then begs you to tell him what all transpired.

Rhuul eyes the helm and Sihir suspiciously.
"Powerful helms don't normally just lie around the mountaintops." he points out reasonably enough.
"And that you don't seem willing to have the rest of us check it for curses or bad effects... is also worrying."
He grins. "I was aboard a ship once. And let me tell you, ships are no laughing matter..."
Those in the know might already be risking the ash and seeking cover.
"And I met this beautiful woman. Well, she had a beautiful rock on her finger anyway. Women without beards... its just unnatural if you ask me. I digress. Noticing me noticing it, she let it catch the light 'Beautiful isn't it?' she said. I naturally enough agreed. 'It is the Wendingo diamond, but it comes with a terrible curse...' she elaborated. Obviously I inquired about the nature of the curse." he pauses for a moment timing the inevitable punch line.
"She sighed dramatically and replied 'Why, Mr Wendingo of course.'"
And casts Break Enchantment on the helm, with an intent to break any curse or enchantment it has over Sihir.

Old Pak frowns when Sihir points out the mempo, then looks on curiously as Rhuul tells his story. When Rhuul finishes his spell, Sihir suddenly realizes what he has done and rips the hat off of his head. Old Pak smiles then. "Ah, well, no harm done ... or, at least I hope not ... ". He quickly rummages through his gear and pulls out a box with sigils carved along it. "This should do, just drop that in there, yes.
This should prevent it from gaining any further advantage over you while you deliver it to the Lodge."
As Narika mentions Old Man Mountain, Old Pak gets another excited look. "Really? You actually spoke to the spirit of the mountain? And it aided you? Oh, that is amazing, I want to hear all about it!"
He then turns his attention to Fossilblight. "And that, of course, is the reason for the adventure in the first place. Good to see the eruption wasn't for naught. Let us hope it was a worthwhile price for what we received!"
