To me, it's pretty obvious the discs are there to help somehow. I don't know how, so I'm guessing that they'll imprison it again.

Illuthion curses and rushes onward, calling on further power to allow him to run even faster. Lets hope it ain't payin' attention to the ceiling!

Aelfward removes both discs from the wall, leaving each sitting on the floor in front of its new slot.
Illuthion runs upside-down across the ceiling, taking a tail slap for his trouble, but avoids the dangerous grapple.
Kyra moves next to Aelfward and buffs the brave warrior.
Tsishad tries in vain to escape the xacarba's hold, but Blub Blub and Periel each hit the creature once--and its rage is quite obvious at Periel's attack. All three tails turns on the paladin--releasing Tsishad--but none manage to hold onto him, and one misses completely.
Everybody is up.

@Illuthion: Both glow, one very brightly and the other twinkles with a dim light. Mechanically, it doesn't matter which you grab first, just how you match the discs and slots.

Aelfward slides the light disc into the "MOON" slot, while Illuthion picks up one of the others. Kyra channels Sarenrae's warmth again, healing her allies.
Tsishad's valiant effort shows how little the thought counts sometimes, as he ends up right back in the xacarba's clutches.
Periel heals up the rest of his bruises, then delivers another devastating blow to the serpent.

The important thing being that, grappled condition or not, it's still a standard action to maintain, so that tentacle could not attack again in that same round.
Another important factor being it can't maintain more than one Grapple at a time.

Is all of this really about the fact that when speaking to myself I said "Tail 3" instead of "Maintain"?
And it does have constrict, I just haven't been rolling it. There are a couple of contradictions in the stat block, and part of the way I resolved them was by foregoing the constriction.

Illuthion grabs the brightest disk, and runs back through the amalgam of golem and creature, shouting as he does: "I AIN'T SURE WHAT THE FLAMING F+~* YOU THINK MY S%~# F&%!ING SWEARING IS GOING TO F+!%ING DO BUT F%%$ING F&$+ ON A F@$%-STICK, THIS S$!% BY CRAY YO!"
Standard to pick up a disk, move all the way through Spicy McHaggis' space, drop it by Aelfward's feet as a free. Cry as a swift.

Illuthion deftly spins to avoid a tail by centimeters, bringing the last disc to Aelfward.
At this point there isn't anything the xacarba can do to stop you. It's just a question of how much damage you take next round, and nobody is within 2d8+4 of dying. Even if I hit Periel three times for max damage, Kyra and his LoH would easily keep him alive.
So . . . agony combat over.
