Now out of elemental rays Allyph tries to think of another way to deal somewhat decent damage, which she finds a bit difficult with the woman/monster 20 feet up in the air. She has her bow, but she's not that good with it.
Allyph lays a hand on Mavor's shoulder and casts vanish on him, making him a bit harder to hit while he's blindly swinging his weapons around. She then figures she'll pick up her dropped rapier.

Baer hits Lady Andaisin hard with her staff, but her second attack is dodged. Baer then takes some distance from the undead woman, also bringing herself closer to the floor.
Denario throws another acid imbued card at Lady Andaisin, but misses. He meanwhile tells Mavor, who's stowing his greataxe and picking his whip back up, what's going on.
Quorthos knows the woman has transformed into an undead, but knows nothing about this kind of undead. Unable to come up with anything else he leaps into the air and stabs Lady Andaisin. (No SA)
While the life link between Tural and Baer restores some of Baer's health, Tural keeps up his chanting and picks up the healing wand he dropped.
Allyph lays a hand on Mavor's shoulder and casts vanish on him, then picks up her rapier.

@Denario: There's no mention in the spell's description of the effect stopping when the person affected passes out.
Denario throws a card at Lady Andaisin, but it doesn't pierce her newly gained natural armor. He then requests a vanish spell to be cast upon him by Allyph, which she does for him. Allyph then moves toward Baer's position.
While Mavor jumps up and blindly swings his whip around, popping back up from invisibility, Quorthos hits Lady Andaisin with a vial of alchemist's fire.
Before she passes out Baer hits Lady Andaisin hard against her head, meanwhile casting infernal healing on herself. She then descends a small distance before passing out and collapsing onto the floor.
Tural casts his fortune hex on Mavor, using his chant to prolong the duration of the effect.
Lady Andaisin looks from Mavor to Quorthos. "I'll get to later, blind fool! First I'll kill this kid who sees me and still doesn't know his place!"
She lowers herself five feet and attacks Quorthos.

Two things: 1) I think Baer is still floating up 10' and will be so for a bit longer. Of course, her long arms also dangle close to the ground. 2) Not sure where Baer got the extra 6 hp. She was down 4 at the start of her ferocious turn, and infernal healing brings that up to -3 that turn, and -2 now. If I'm wrong, happy to correct.

@Baer: 1) The fly spell states that 'using a fly spell requires only as much concentration as walking.' That's not a lot of concentration at all, but it is something you have to be conscious for. The spell's still in effect, in case you want to fly again.
2) Tural's life link continues to heal you for 5 HP each round, even when you're unconscious. The life link's only severed if Tural or the recipient dies, or Tural severs the link as an immediate action. So you're currently conscious and can take actions.
Quorthos jumps up and stabs at Lady Andaisin, but his arm's slapped aside. Meanwhile the alchemist's fire he threw earlier continuous to cause burn wounds.
Denario moves toward Baer while taking out a healing potion.
Tural takes a step toward the party and cures himself with the healing wand.

The fly spell gave you the ability to fly, just like you already had the abilities to walk and swim. It doesn't magically keep you afloat, it magically added flying to your repertoire of 'modes' you can use to move around.
Like the spell's description mentions flying requires as much concentration as walking; it's something you have to do consciously. The moment you went unconscious you could no longer concentrate on flying. Just like a bird that would somehow lose consciousness while hovering you dropped straight to the floor, because you weren't able to concentrate on the act of flying.
The fly spell is still effective so now you're conscious again you could opt to fly again. (Considering you're prone but can fly, I'd say you can use your 5-foot step to fly back up just enough to place your feet below you if you so wish. Just throwing it out there in case you'd like an option to get up from prone without provoking.)
The enemy's 10 feet north of you, once more above the fountain. I also dropped a large image of Lady Andaisin's new form to the side of where you're fighting on the map, so you can see what she now looks like! :)

Quorthos grits his teeth, leaps, and stabs Lady Andaisin with his dagger.
Allyph shoots an arrow into the ceiling.
There's a look of sheer frustration on Lady Andaisin's face when she notices Baer's back up, but Quorthos's taunt causes her to continue to focus her attacks on him.

The man, who you recognize as Blackjack, gracefully lands on the edge of the fountain, next to where Quorthos lies. "Word reached me through one of our mutual contacts, who you'd sent a note about your struggle with this evil. I have come to aid you, saviors of Korvosa!"
Lady Andaisin lets out a frustrated cry. "Then you'll die with them!"

Tural looks at his body and judges that one more round without healing would be safe. After taking a 5 foot step forward he starts pounding his feet on the ground and clapping his hands in the beat of his chant, encouraging the group to go straight ahead into the attack.
Chant is up, prolonging fortune on Baer and Mavor. Battlespirit is now also up, giving everyone +1 on Atk and Dmg rolls.

While Tural continues to chant and activates his battle spirit, Quorthos hits Lady Andaisin with an orb of acid and draws a dagger.
Denario pops up from invisibility, but not before he's thrown a card that deals massive electricity damage to Lady Andaisin.
Mavor jumps up and finally gets a hit it, cutting deeply into the monstrous woman's torso. With the aid of Tural's fortune hex he lands safely.
With the aid of Tural's fortune hex Baer breaks several of Lady Andaisin's ribs. She follows up with an attack that snaps the woman's neck.
Lady Andaisin drops to the floor. Her body starts hissing and melting, and soon it's gone. All that reminds you of Lady Andaisin's presence are your wounds and the objects she carried.
Combat's over!
Blackjack sheaths his rapier and jumps off the fountain's edge. He holds his hand out at Quorthos, to help him up.
"Well done, saviors! You have rid Korvosa of a great evil. Had this monster roamed the city it would certainly have cost many lives."

Denario grins, and flips the remains the finger.
"No way!"
He picks up one of the symbols, and casually flips it in his hand a couple of times.
"This aught to be some proof for the hellknights to investigate the matter."
He tosses it at Blackjack.
"Maybe you've got some 'friends' who could make it happen. We're on the wanted list after all, unjust as it may be."

"Blackjack? I knew the voice sounded familiar. But Denario... Blackjack is also on the wanted list, just like us. Only he has more love from most citizens than we have."
Mavor stumbles off the ledge of the fountain and seems a bit lost as to where to go.
"I guess we need to go quickly. Someone must've alarmed the guards by now. I'll need a hand or something to move more quickly though."

"Exactly. He's alone, and given he's snuck in here, he likely has a good chance to discretely leave something where it needs to be found."
Denario walks over to Mavor, and places Mavor's hand on Denario's own shoulder.
"There we go."

Baer continues to beat upon the oozy flesh of the decaying priestess until her breath gives out. She had found a new well of rage and anger in her brief time asleep and uses it all upon the already dead thing.
"I DON'T LIKE YOU!" Baer screams as her breath becomes more ragged. Finally, the beating stops and she is able to look around while still panting.
"I think I was piddlespotting seeing things...like...Blackjack?" Baer turns to regard the man. "Blackjack? Is that you? Really you?"
Baer stares at the man, waiting some sort of explanation.
"How did you even get in here? We...we have the elevator key! Wow...you're amazing!"

Tural drops his spells and slumps a bit from exhaustion.
"Glad that's over with. Baer, Quorthos you alright?"
As he starts to walk over he notices the wounds on their body are disappearing and he starts to feel worse and worse. He quickly starts healing himself to not go down.

Quorthos takes Blackjack's hand and rises shakily to his feet, his formality belying his awe of their rescuer. "Your reputation of arriving in impossible situations in the nick of time is well-deserved, sir. Thank you." He shakes the man's hand sincerely.
"I think we should probably send this unholy relic and a note to Kressida - it's probably good for us to provide her with the "glory" of the discovery. " He fills Blackjack in on the relationship with Kressida, and our belief that she stands with Korvosa beyond her rank.
Quorthos helps gather up the loot, and then encourages everyone to vacate the premises before the Grey Maidens show up. On the way out, he stops to administer antiplague to any civilains still here, and frees any from their bonds. (Sorry, at work and can't easily check back to see if we already did this.) If there are any of his 12 left, he takes one himself, concerned about the Strength loss.)
