How deep is the well? Is the ledge wide enough to stand on? Rope and bucket? Perception: 3 + 7 = 10.
There is a circular brick wall about two feet high surrounding the well, with a rim two bricks wide – about 8 and a half inches. There is a rope and a sturdy wooden bucket. The moonlight reflects off water some way down, but you can’t tell how deep it might be.
The grasping weeds and grasses fail to find purchase on the spider’s spindly legs. It scuttles forward to bite the wolf as the beast howls, but its fangs appear not to find purchase in the wolf’s hide.

The disk hovers three feet above the ground, so I’d say that it would take a move action to climb up onto it. Keep in mind that the disk winks out of existence if you try to take it more than three feet away from the surface beneath it, as per the spell description.
You could use it to safely move over the entangling weeds, and to give yourself a 'boost' in climbing onto the cottage roof, but not as an elevator to lift yourselves up there.
Keildrithe stands his ground on the well ledge, awkwardly and nervously waving the silver knife in one hand, the sprig of dead wolfsbane in the other.
Balance check: 7 + 1 = 8. Readying an action -- should the werewolf attack him, he will try to lure/push it into the well... unless the poor balance check makes Keildrithe fall into the well???
After casting the spell, Laithe circles down towards Kajetan's position. to try and get closer to his companions. He still can't see the wolf properly through the darkness and the distance, but judging from the reaction from his friends he guesses it is big, and starts to doubt whether his entangling vines will have any affect on it.
With last action moves to T14
When he see's Keildrithe take up position at the well, he transforms into wolf form and moves closer, but staying in cover of a nearby bush, hoping to stay hidden but close enough to aid his kinsman if he is the one the wolf heads for.
Moves to W21 and will attempt to hide if he can. Stealth check:
Round 5, 6 and 7 have become a bit jumbled up, but I think I’ve got it sorted out now. Summary of round 6 actions posted below. To make it all work (and to account for the wolf being dazed) I’ve dropped it down the initiative order. I believe everyone but the wolf has acted in round 6.
Init Round 6:
Keildirthe: 21 (round 6 action: hop up onto rim of well)
Nicolai: 20 + (round 6 action: cast floating disk and climb on
Laithe: 20 (round 6: turns wolf, moves to W21 and attempts to hide)
Kajetan: 17 (round 6 action: retrieves scroll and begins casting summon swarm)

Alton: 13 (round 6: climbs up onto floating disc)

Keildrithe jumps up onto the rim of the well, but has trouble balancing on the narrow surface (based on your balance check result you will either need to use one hand to steady yourself – dropping either the knife or the wolfsbane in the process – or drop harmlessly back down to the ground beside the well).
The great wolf growls low in its throat, then lunges at the oversized spider through the doorway, its jaws open wide.
Attack: 1d20 + 7 ⇒ (19) + 7 = 26 Damage: 1d8 + 7 ⇒ (6) + 7 = 13
It's sharp teeth tear a huge chunk out of the spider's cephalothorax. It worries at the wound, but is unable to trip the many-legged menace.
Having fallen back off the ledge of the well, and determined not to drop the silver knife or the wolfsbane, Keildrithe tries again to clamber onto--and keep his balance--on the ledge on the side of the well that is furthest away from the cabin. He trembles as he hears the wolfman savage the equally frightful spider that materialized out of thin air but a moment ago..
"Laithe, to me," he calls out. "Together, perhaps we might be able to lure and push it into the well, then keep it trapped down there until it turns human again?"
Balance check: 18 + 1 = 19. Might be a dumb idea... but perhaps Laithe would consider readying an attack, then charging/bullrushing the werewolf if it should attack Keildrithe. Depending on how the sequence of events go, the two Karands might be able to aid one another?
At Keildrithe's call, Laithe-Wolf creeps forward a little more, remaining in cover of the bush but with a clear line on the well ahead of him. He crouches low to the ground, ready to charge if the wolfman approaches his Karand brother.
Would someone like to take Kajetan's turn? He was casting summon swarm, and can then take a full action after that. Also, don't forget the summoned giant spider.
The giant spider's mandibles clack angrily, yet fail to pierce the lycanthrope's flesh.
Attack: 11 + 2 = 13.
Yet soon, a swarm of coin-sized spiders emerge from the grass and start to crawl towards the werewolf.
Summon swarm range = 35 feet... so focused on R17 or so, assuming Kajetan is standing in T11? If the swarm can move the same round and attack the werewolf (presumably the nearest creature), Damage: 3; distraction; Poison save DC 11, or suffer 1d2 points strength loss.
Kaj crouches low in the wolfsbane patch, trying to maintain his concentration and willing the swarm--which seems to have a 'mind' of its own--to prey on the werewolf.
If he still has a full move action left, he would probably cast Spectral Hand (to prep for spells like touch of fatigue and vampiric touch) and probably look for better cover?
EDIT-Sorry, you said a Full ACTION, not a full ROUND? I would vote run for cover, following the others around the cabin!
Hearing the roar of the (likely enraged) beast, Nicholai glances towards the front of the cabin.
Concentrating to maintain a spell is a standard action. No checks are required under normal circumstances (caster is not taking damage, distracted etc).
Note that the swarm summoned by summon swarm cannot be controlled in any way by the caster – concentration merely maintains the spell. The swarm will move towards the nearest creature.
Now, if summoned at R17 – which is at the maximum distance Kajetan can cause the swarm to appear – there are several creatures closer to the swarm than the wolf – Kajetan, Laithe, and the summoned spider are all closer, and several sheep are certainly closer. As I haven’t been telling you (or keeping exact track myself) on which sheep have pulled their stakes and fled, I’ll rule that all the sheep outside the entangling area have done so by now, but I specifically said earlier that the sheep inside that area had not fled and were entangled. If you can eliminate these sheep they will no longer count as ‘creatures’.
Although there is no specific exclusion, I will rule that diminutive creatures are not entangled by an entangle spell. However, the area of the entanglement still counts as difficult terrain which will affect their movement.
Kajetan completes his spell, and hundreds of tiny brown spiders begin boiling up out of the grass. Despite Kajetan mentally willing them towards the wolf, the spiders begin heading towards the nearest sheep, bleating with terror as it is held fast by the entangling weeds.
Spider swarm summoned into R17, R18, S17, S18. It moves to N17, N18, O17, O18 and attacks the sheep there. I’ll give the sheep the same stats as a goat but without the gore attack. It takes 3 hit points from the swarm and has 2 remaining.
Kajetan keeps concentrating on his spell, hoping that it might yet do some good.
Alton and Nicolai clamber from the floating disk up onto the timber shingled roof of the cottage. The wolf – still trapped within by the spider – raises its head to the roof above it momentarily, then continues to focus its attention on its eight legged captor, once again lunging through the open doorway at the arachnid.
The massive wolf tears through the spider's carapace. The giant arachnid drops to the ground, then vanishes as the magic tying it to this place is severed.
Nicholai's eyes go wide as he sees the jaws emerge from the doorway and tear the spider in half. He opens his mouth to say something clever, but settles for a loud gulp instead.
Moth, is the roof sound and solid? Or are there chinks in it? something we can peek through or drop a coin-sized object through? I'm working in a thought. Keep the wolf/beast distracted as long as possible. a light spell or a ghost sound spell might help here? :)[/ooc]
Like Nicolai, Keildrithe swallows hard but stands his ground on the edge of the well that is furthest from the cabin (i.e., so that the well itself lies between Keildrithe and the cabin). He continues to hold the silver knife in one hand, and the sprig of dead wolfsbane in the other.
"Behind me, Laithe, behind me," says the bard softly, a tremor in his voice.
Readies to attack the werewolf, should it come within 5' of him -- if the werewolf doesn't become distracted by the spider swarm, hopefully it will leap at Keildrithe over the well if it <gulp> does attack him? The ideal would be for Keildrithe and Laithe to lure/push the werewolf into the well and to then cover the well with the floating disc... don't know if we can pull that off, though. In the meantime, distractions like ghost sounds spells are good!
Seeing Nicolai searching for a gap in the roof Alton, feeling somewhat helpless at the moment, grasps one of the old shingles on the roof and tears it free.
Nicholai takes the shingle from Alton, casts a Light Charm on it, and drops it into the hole Alton made by tearing it up. He makes a gesture of silence to the shorter man, then peeks into the hole to see what the wolf is doing,...
At this point I'm just trying to keep it distracted! (ooc I'm worried about the tangle spell running out of time, but I figure it's better(?) to keep this thing IN the cabin if at all possible!) :/
Hmmm... Maybe it would be smarter to get Kajetan to run in the opposite direction from the swarm after all? I would hate for it to turn on him. Maybe he should run for the woods, and try to come up somewhere behind Keildrithe and Laithe?

For the record I would of summoned the swarm on top of the werewolf, moving closer if required.
Kajetan continues concentrating on the swarm, cursing himself for summoning it in no mans land. He takes a slow cautious move towards the well, hoping to add himself to the bait list.
To be honest, I didn't think Kajetan would want to risk getting closer to the werewolf -- I thought he would have preferred to stay hidden in the wolfsbane patch. Sorry for misjudging.
